Summary:
Provides occupational therapy examination and evaluation to determine prognosis, provide intervention, and update plan of care.
Assists manager/supervisor with clinical and operational issues. Works with manager to improve processes and utilization of rehab services.

Position Responsibilities:
Consults with medical, ancillary professionals and other team members/supervisors to provide optimum quality patient care and effective operations

Demonstrates comprehensive evaluation skills and interventions. This includes evaluating the effectiveness of interventions and modification of plan of care as appropriate

Maintains and completes evaluations, patient records, reports and all written documentation effectively and in a timely manner, in accordance with regulatory and department guidelines

Effectively utilizes aides and other support staff as appropriate

Effectively communicates with patient and/or family/significant other in providing education and literature in accordance with the patient's age, physical condition and cultural needs

Position Qualifications Required
Experience Required:

Required Education:
Minimum of Bachelor's degree in Occupational Therapy from a program accredited by the AOTA.

Training/Certifications/Licensure:
Current licensure in the State of New Jersey.
CPR certification.
For home care and early intervention staff, a valid driver's license, current automobile registration and proof of insurance.

Home Care Clinical Nurse Specialist WOC (ET) Nurse Part time - (Mt Laurel, New Jersey)

Role Statement:
Responsible for assessing, diagnosing, planning, implementing, and evaluating patient care needs with selected disorders of the gastrointestinal, genitourinary and integumentary systems through outcome identification and evidence-based practice. Coordinates patient care and communicates and collaborates extensively with healthcare team, patients and/or patient's family or significant other(s). Maintains and demonstrates high standards of professional ethics.

Job Essentials:
- Performs an gastrointestinal, genitourinary and integumentary assessment of the patient, leading to development, implementation and evaluation of a plan of care
- Administers, monitors and documents therapeutic interventions and regimes related to wound, ostomy or continence.
- Effectively manages rapidly changing situations.
- Participates in clinical decision-making.
- Educates patient and family/responsible party regarding disease process, individual care needs, wellness, safety issues, etc.
- Delegates and monitors care rendered by other members of nursing department and healthcare team.
- Demonstrates standard precautions and patient safety principles in practice.
- Participates in orientation, education and development of healthcare team.
- Identifies and participates in the performance improvement activities.
- Maintains professional and departmental level competencies.
- Collaborates with multidisciplinary team to achieve highest clinical quality

Educational Requirements, Qualifications:
Must be a graduate of an accredited school of nursing and have valid NJ RN License. BSN required. Current BLS. Current board certification as WOC (ET) nurse (CWOCN) by the Wound, Ostomy and Continence Nursing Certification Board or presently enrolled in WOCN accredited ET program with certification within 12 months of hire. Refer to the Educational/Regulatory Requirement (Department Specific) policy in the Nursing Administrative Manual. Demonstrates knowledge of nursing skills, hospital practices, procedures and standards. Perfects skills and stays abreast of current nursing practice through on-going education, journals, etc. Must have strong communication skills. Must be able to coordinate efforts of a team of care providers. Must have current proof of driver's license and auto insurance if position requires travel within hospital system.

Supervises the Home Health Aide in the home. Promotes delivery of quality care and efficient utilization of agency resources.

Job Essentials:
- Conducts comprehensive patient assessments incorporating psychosocial and physiological status as well as health behaviors, compliance with therapeutic regime and technical procedures.
- Coordinates plan of treatment with the physician through timely and appropriate communication with the physician.
- Administers, monitors, and documents therapeutic interventions and regimes.
- Demonstrates complete knowledge of established standards for documentation , use of point of service devise, and time frames for completion.
- Educates patient and family / significant other regarding disease process, individual care needs, wellness, medications, safety issues, etc.
- Collaborates with other members of the healthcare team to establish the plan of care
- Maintains standard precautions and patient safety principles.

Educational Requirements, Qualifications:
Must be a graduate of an accredited school of nursing and have a valid NJ RN License. Baccalaureate Degree preferred. 1 - 2 years recent work experience in a hospital i.e. Med/Surg, E.R., OB/GYN, Peds, MICU, CCU or other specialty units and/or one year recent community health experience. Broad knowledge of nursing practice and ability to apply specific skills in the home health setting. Homecare exp. 1-2 years.
