<description><![CDATA[Summary:<br/>Provide Triage Assessments in person and on the telephone with patients to determine the appropriate level of care required. Prepare and assist clinical providers with patient examinations and procedures. Perform clinical tasks within the scope of their licensure. Leads team as appropriate and assists with assignment of tasks.<br/><br/><br/>Position Responsibilities:<br/>Conducts Triage patient assessments incorporating psychosocial and physiological status as well as health behaviors, compliance with therapeutic regime and technical procedures. Demonstrates a consistent ability to perform patient triage via telephone as well as in person. Identifies and evaluates patient and family needs, abilities and readiness to learn via telephone or schedules appointments. Instruct when possible teaching needed via telephone. Demonstrates complete knowledge of established standards for documentation. Communicates effectively and professionally with staff, supervisor, clients, physicians and others involved in care. Handles in office and telephone emergency situations. Stat lab/procedure follow-up. Facilitates medication refills per office policy. PT/INR follow up. Orders office medications/vaccines<br/><br/><br/>Prepares patients for exam by obtaining and documenting vital signs, chief complaint<br/>Administers vaccines per protocols and standing orders prior to physician exam.<br/>Performs specimens for processing. Assists with Minor surgical procedures and general medical treatment such as administering medication, applying appliances/dressings, performs first aid for injuries, lacerations, punctures, etc. in accordance with policy. Administers and documents medications requiring calculation and follow up assessment as directed by the physician. Performs diagnostic tests such as EKG, Pulmonary Function Testing ,audiograms, vision screening. Provides patient education such as diabetic teaching, use of inhalers. <title><![CDATA[Nurse Practitioner - (Voorhees, New Jersey)]]></title>
<description><![CDATA[<p>Will independently assess, diagnose, prescribe medications and treat patients according to the established protocols in collaboration with physicians.  The NP will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses' Association Code of Ethics. Provide triage referral coordination, develops differential diagnosis, and individualized care plans along with patient education for specific disease types as appropriate.<br/><br/><br/>Position Responsibilities:<br/>Provides the range of treatment or service needed by the patient. This range of treatment  includes, but is not limited to History and Physicals exams, consultations and referrals, Ordering and interpreting laboratory and diagnostic tests.  Pharmacologic interventions, where clinically indicated, will be in accordance with the established joint protocols and APN prescribing guidelines.<br/><br/>Performs therapeutic treatments, including but not limited to: wound care, suturing, performing and assisting with minor office procedures, and drain removal.<br/><br/>Administrative responsibilities which may include scheduling surgical cases, providing triage, orientation of clinical staff, precepting of students, and call rotation as required based on the practice. Ensures practice compliance in accordance with regulatory guidelines. Displays motivation and initiative in meeting the goals of the practice.<br/><br/>Appropriately documents subjective and objective findings including assessment and plan in the patient&iquest;s medical record. Ensures accurate coding and billing.<br/><br/>Collaborates with interdisciplinary care team to provide each patient and their family the highest quality healthcare. Including assessment of patient/family knowledge and initiation of patient/family education. For uncommon or unstable conditions, the NP will participate in the diagnosis with consultation and either refer to a specialist or participate in the dual management and treatment with the collaborating physician.<br/>Position Qualifications Required<br/>Experience Required:<br/>Must have excellent clinical judgment, communication, and interpersonal skills.<br/><br/>Required Education:<br/>Graduate of an accredited School of Nursing, Masters Degree in Nursing.<br/><br/>Training/Certifications/Licensure:<br/>Stay current with Medical trends and provide evidence-based practice medicine by completing the required continuing education credits as mandated by state requirement.<br/>Certification as an Advanced Practice Nurse is required. <br/>Current NJ RN license and appropriate Advance Practice licensure for specialty.   <br/>BLS certification.
